Team Roster Guidelines:
1. All names must be complete (no nicknames).
2.You will still need to sign either the
singles and doubles
or
team
assumption of risk form. Please either print this one or pick one up from Dustyn Miller in Funkhouser.
3. Additions or deletions from team roster may be made prior to the second scheduled contest.
4. Incomplete rosters will not be accepted. Teams will be suspended from the sport for giving false information on the entry form.
5. You will be notified by email of your upcoming tournament schedule. Schedules will be put in your boxes after the entry deadline. Please notify the Intramural Office of your results by phone call or email as soon as you complete your game or match.
