International Education Waiver Forms
Bridgewater College encourages its students to take advantage of international travel experiences. These are required forms for participants of international Interterm programs.
Complete and submit the appropriate online Agreement, Release, and Emergency Information form by Friday, November 15th, 2013.

- Agreement and General Release Form,
- Informed Consent/Privacy Act Waiver
- Privacy Act Waiver for U.S. Department of State
- Mandatory Medical Insurance Agreement
- Cancellation and Refund Policy Agreement
- Emergency Contact & Medical Form
Mandatory Insurance: All participants in Interterm travel courses who are under the age of 64 are enrolled in an HTH medical and evacuation policy. A copy of policy can be found here.
Cancellation insurance: We understand that international travel experiences can be expensive, so we encourage students to reduce the risk of travel interruption losses to the extent possible. Each student is responsible for obtaining travel interruption or cancellation protection, on a voluntary basis. There are multiple sources of insurance products to cover such events as illness or family emergencies that might result in the loss of trip deposits or other expenses. Should you need help identifying some plans, you are encouraged to speak to your currant insurance representative or contact the Center for International Education at StudyAbroad@bridgewater.edu.
