Media Release Form

For the sole consideration of my participation in the activity described below (the “Activity”), I, the undersigned, grant to Bridgewater College (the “College”) the absolute right and unrestricted permission to copyright, use, publish, republish, and distribute my likeness and voice in photographs of me (still, single, multiple or moving), audio, and video (digital, electronic or other medium) (collectively, all such likeness of me and reproductions of my voice in all such media, the “Materials”). This permission extends to the publication of the Materials in physical, tangible format, as well as to the use of the Materials on social media, the College’s website, and other electronic and Internet platforms.

I waive any right that I may have to inspect and approve the Materials. I further agree and understand that the College may reproduce and distribute the Materials in any medium in promotional and other publications the College deems appropriate. I do hereby release to the College all rights to exhibit the Materials publicly or privately, and assign to the College all copyright or other intellectual property interest in the Materials, which shall be the sole and exclusive property of the College. The College may also assign its rights to the Materials.

I waive any rights, claims or interests I may have to control the use of my identity or likeness in the Materials. I agree that any uses described herein may be made without compensation or additional consideration to me, and I waive any right to royalties for the use of the Materials. I further consent that my name and identity may be revealed in the Materials or by descriptive text or summary.

I hereby release and forever discharge the College and its students, officers, agents, and employees from any and all claims, demands, rights and causes of actions of whatever kind that I might be assert against them, arising from the use of the Materials.

I affirm that the only consideration for signing this Release are the above-stated terms, that no other promise or agreement of any kind has been made to or with me by any persons or entity whomsoever to cause me to execute this Agreement. This release is a necessary precondition to my participation in the Activity. I understand that if I do not sign this release, the College will not allow me to participate in the Activity described below.


IN SIGNING THIS RELEASE, I ACKNOWLEDGE AND REPRESENT THAT I have fully informed myself of the contents of this release by reading it before signing. I understand its contents and binding effect, and I am signing this document freely and voluntarily. No oral representation, statements or inducements have been made. I further state that I am at least eighteen (18) years of age and fully competent to sign this release. If the participant is a minor, his or her parent or guardian must sign this form.
